Reminder! Submit your Vaccination Attestation/Exemption Form

The following message from the Office of the Registrar was distributed directly to students to their @upei.ca emails.
Dear Students,
Were so excited to have so many of you back on campus for the 2021 Fall Academic Semester!
On August 31, 17勛圖 updated its process for the 17勛圖 Mandatory COVID-19 Vaccination Policy and issued a message to students, faculty, and staff. Read the full message: /communications/news/2021/08/proof-vaccination-now-required-part-upei-covid-19-vaccination-policy
All members of the University community (faculty, staff, and students) who are going to be on campus from September to December were asked to fill out the mandatory Attestation and Exemption Form at , regardless of their vaccination status. If you have already filled out the form, we thank you, but if you havent, please do so as soon as possible!
In the form, you are asked to declare that you are either:
- fully vaccinated with a Health Canada-approved vaccine, and then you can upload an image or file (up to 20 MB) of your proof of vaccination.
- partially vaccinated with a Health Canada-approved vaccine, and then you will be asked to indicate when you expect to receive your second dose.
- not vaccinated with a Health Canada-approved vaccine, and then you can indicate when you expect to receive your vaccine, or that you require an exemption for medical, religious, or cultural reasons.

The 17勛圖 Health and Wellness Centre will begin weekly COVID-19 testing soon, which is required for those who are not fully vaccinated. Please watch Campus Notices in the student portal to learn how to register. Those who are not fully vaccinated must also wear a mask at all times. 17勛圖 has also mandated the wearing of a mask in indoor spaces on campus as physical distancing of 2 metres/6 feet will be difficult to maintain in most campus settings: /communications/news/2021/08/upei-re-introduce-mandatory-masking-effective-september-8-2021
International students are reminded that if they are not eligible for a PEI Pass, i.e., not vaccinated with a Health Canada-approved vaccine, they will have to self-isolate for a total of 14 days upon arrival to PEI to fulfill federal and provincial government requirements. Students already in Canada who are not eligible for a PEI Pass are required to self-isolate for eight days upon arrival to PEI. Failure to comply with self-isolating guidelines will result in consequences.
